If you or your spouse have 40 work quarters (~10 years), Part A is premium-free. 30-39 quarters get reduced rates. Less than 30 quarters pay full premium.
All U.S. citizens and eligible permanent residents aged 65+ can enroll in Medicare. Part A is free for most people with work quarters. Parts B/C/D require monthly premiums. High-income beneficiaries also pay IRMAA surcharges.
IRMAA is based on your MAGI from two years prior. For example, 2026 IRMAA uses your 2024 tax return. Social Security notifies you annually if you owe IRMAA.
Part C (Medicare Advantage) is a private insurance alternative that combines Part A and Part B benefits, often including Part D. Premiums vary by plan and region.
This calculator uses CMS-published 2026 standard rates and IRMAA tables for estimation. Actual premiums may vary based on individual circumstances, region, and plan choice. Final amounts are determined by CMS notification.
